POSITION: Housing and Programming Case Manager - Hourly Non-Exempt
Pay Range: $18-20/hr.

The position of Housing and Programming Case Manager will schedule, coordinate and oversee all aspects of resident programming at ACCESS under the direction of the Director of Resident Services. They will be responsible for maintaining a caseload of residents where Individual Service Plans are developed with the goal of securing housing and income. Additionally, they will provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of the residents’ needs. This is a full-time hourly position with fluctuating hours defined by the activities and programming at the Shelter.
QUALIFICATIONS:
Completion of bachelor’s degree in Human Services or related field and a minimum of six months experience in social services. Communicate effectively in oral and written form with residents, family members, agency staff, other social service agencies, and the public; as well, strong active listening and cognitive skills. Ability to gather, collate and classify information about data, people, or things; interpret variety of instructions in written or oral format; deal with problems involving several variables within unfamiliar context; write meaningful, concise and accurate reports, define problems, collect data, establish facts and draw valid conclusions; counsel on sensitive matters. Basic knowledge and understanding of the challenges facing the homeless populations. Valid Ohio driver’s license and proof of insurance.
Security Duties:
1. Responsible to oversee, coordinate and schedule on-site resident programming.
2. Meet with assigned adult resident within 48 hours of arrival at the emergency shelter.
3. Develop Individual Service Plan for each resident within 48 hours and assist residents in achieving goals outlined in their Individual Service Plan. Objective is for resident to identify, obtain and sustain independent housing.
4. Maintain resident files, case notes, agency forms and other documents within assigned deadlines.
5. Gather personal items from shelter closets including clothing, shoes, coats, and any other personal items to meet the needs of the resident.
6. Collaborate with community agencies to assist residents.
7. Assist residents in identifying community and personal resources and discuss the programs available to them.
8. Maintain case notes to document resident interactions and submit weekly updates.
9. Act as the Ohio Benefit Bank Counselor and complete the necessary paperwork to sign residents up for benefits as needed.
10. Enter and maintain records into HMIS, updating resident photo sheet and providing HMIS reports as needed.
11. Making corrections to HMIS Data Completeness Report monthly.
12. Comply with housing services best practices.
13. Practice HOUSING FIRST model.
14. Collaborate with the Resident Services team to prioritize the residents’ needs for in-house training and education, as well as identify outside resources to maximize the success of their “ISP.”
15. Assess resident’s needs and developments of new programs/classes.
16. Maintain monthly calendar of classes/events.
17. Track the ongoing activities of the in-house programs; maintain accurate and easily retrievable records of resident training, programming, attendance, evaluation, and review.
18. Attend staff meetings as scheduled.
19. Deliver any required reports on a timely basis.
20. Represent ACCESS to community resources as required.
21. Responsible for other duties as assigned by the Director of Resident Services.
22. Responsible for adhering to all policy and procedures as outlined in the employee handbook and other departmental procedure manuals.
ACCOUNTABILITY: The Program Case Manager position reports directly to the Director of Resident Services.

· While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
